Self-Advocacy Coaching
Shifting our life story to include supports as we age is not an easy task. Rather than seeing supports as an invasion of privacy, or a sign that someone is "declining," we view supports as testaments to self-advocacy. Using a walker, or installing safety bars, or wearing an emergency call button, are acts of affirmation of our resolve to address needs as they arise.
Assessment and Monitoring
Sometimes an informal assessment (call it an attentive conversation) is the best way to get a sense of a person's needs and wants. Whether concerns are mobility, safety at home, cognition, or physical conditions, an assessment provides a benchmark from which to start.
Healthcare Advocacy
Often it is useful to have a second set of eyes and ears at medical appointments. Additionally, making sure that life planning documents (Living Will, Healthcare Power of Attorney (Agent), New Mexico's MOST Form, DNR) are completed / updated and reflect the wishes of the individual is a crucial step towards ensuring appropriate medical supports. Everyone over 18 years of age should have a Living Will and Healthcare POA / Agent document completed, even in good health! You can get the documents for free in the Resources section of this site.
Family Communication
Watching a parent grow older (and growing older ourselves) brings myriad emotions and fears. Sometimes an outside facilitator can serve as an objective family mediator to help sort out conflicts that arise. Listening to concerns and viewpoints free of family history can result in a plan that reflects positive input and change that works. In-person and virtual meetings available.
End of Life Concerns
- Legacy Work: what do you want to communicate to friends and family before you die? What defines your life experience? What is left unsaid or unwritten?
- Obituary Writing: a good obituary is more than a life summary. Highlighting your values and experiences can be a rewarding experience.
- Care coordinator to communicate with your care team, including medical professionals, family near and far, friends, neighbors, hospice services, and more.
- Aesthetic guidance and sensory support in creating an environment that supports your physical, emotional, and spiritual needs.
- Medical literacy and paperwork guidance and completion.
- Grief education, support, and resource referrals.
